Acute kidney injury (AKI) poses a significant challenge in the intensive care unit (ICU), with incidence rates reaching 50–60% among ICU patients. Approximately 10–15% of these cases progress to severe AKI, necessitating renal replacement therapy (RRT). A recent clinical practice guideline, developed by a multidisciplinary panel under the auspices of the Chinese Society of Critical Care Medicine, aims to standardize RRT protocols for critically ill adults. Published in the Journal of Intensive Medicine, the guidelines address key clinical issues such as optimal timing for RRT initiation and modality selection.
The guidelines endorse early RRT initiation in cases of life-threatening fluid overload or acute electrolyte imbalances. Strong recommendations include ultrasound-guided vascular access techniques and regional citrate anticoagulation (RCA) as the preferred strategy for continuous RRT in non-bleeding risk patients. For patients at risk of bleeding, RCA is still recommended, underscoring the importance of tailored anticoagulation approaches.
A total of 34 recommendations were categorized based on GRADE methodology, focusing on various aspects of RRT. This includes fluid management and the potential to wean patients from therapy based on urine output recovery. The diversity of recommendations reflects the complex physiological environment of critically ill patients.
These guidelines are critical for clinicians, as they provide a standardized framework to enhance patient outcomes and reduce AKI-related complications. However, it’s important to note that the recommendations apply specifically to adult patients and individual clinical circumstances may necessitate deviation from guidelines. As such, ongoing research and clinical efforts are essential to refine RRT strategies further and ensure optimal care for this vulnerable patient population.
