Adam Cassidy was born with a rare genetic condition and was baptized in hospital shortly after his birth because his mother feared he would not survive.
He was flown from Altnagelvin Hospital in Londonderry to Birmingham for treatment, where he was diagnosed with alpha-1 antitrypsin deficiency.
He survived but developed complications in July last year and was flown to Birmingham for a liver transplant.
A year later, the 12-year-old girl returns to the city as a contestant in the British Transplant Games.
Adam is planning to enter a swimming contest after being recommended by a consultant.
The event, which begins late Thursday, is set to be the largest ever, with more than 1,000 transplant athletes participating.
“I'm really excited,” said Adam, who has been taking swimming lessons in the lead up to the event.
“It would mean a lot to me, maybe I could win a medal.
